Claim Form Login

If you are or were a patient of Main Line Fertility, Ltd. and/or Main Line Fertility Center, Inc., collectively d/b/a Main Line Fertility, who had a patient portal account with Main Line Fertility as of February 5, 2025, then you may be entitled to Settlement Benefits.

If you are a Settlement Class Member and wish to receive a payment, you must complete a Claim Form online or postmarked on or before December 29, 2025.

Privacy Shield Pro Product: Each Settlement Class Member will automatically receive a code to enroll in the Privacy Shield Pro product.The code is listed on the Notice you may have received.

Cash Payment: Each Settlement Class Member who files a timely, valid claim will also have the option to select a cash payment of $35.00.

To start your online claim, you must login with your Class Member Login ID and Password. You can find your ID and Password on the Notice that was sent to you. If you are unable to locate your ID & Password, please contact the Settlement Administrator by emailing info@MLFSettlement.com. Please request your ID & Password by providing your full name and mailing address.
